[ ] One (1) 2×2 ID Picture
[ ] ATM Card with PIN Mailer/Passbook
[ ] Marriage Contract (Authenticated)
[ ] Community Tax Certificate
[ ] Brgy. Clearance
[ ] Proof of Billing
For the Co-maker
[ ] 2 Valid Identification Cards
[ ] One (1) 2×2 ID picture
